A Victorian ombudsman report found a 16-year-old boy spent more than 22 hours a day in a cramped cell at Melbourne Assessment Prison for six weeks after being transferred from youth justice. The conditions bordered on solitary confinement, prompted suicidal thoughts and breached his human rights, the report said.
