HealthNg Teng Fong General Hospital pilots home care after discharge

Ng Teng Fong General Hospital and social service agency Loving Heart are testing a programme that provides home support to patients with complex chronic conditions after discharge. Retired nurses and trained community carers will help at home, with a possible future expansion across Singapore if the pilot succeeds.
