> Tennessee’s prison commissioner, Frank Strada, will leave his post this month after the execution of convicted murderer Christa Pike failed. Witnesses said Pike was still breathing and moaning after two doses of pentobarbital, forcing officials to halt the procedure and send her to hospital. Executions are suspended pending an independent review.
