> The Charity Commission for England and Wales has formally warned Save the Children after a two-year investigation found governance failures, policy breaches and misconduct at its Yemen office exposed staff to serious risk. The inquiry followed the death of security director Hisham al-Hakimi after his detention in October 2023. Save the Children accepted serious shortcomings and said it had introduced new leadership and stronger safeguards since 2024, while closing its north Yemen office over security concerns.
